Lifelong seeks a Bilingual Medical Case Manager that assists primarily medium to high acuity individuals living with HIV in accessing and engaging in psychosocial, medical, homecare, and supportive services. This position conducts comprehensive assessments, develops and updates service plans, and facilitates the acquisition of all appropriate services. This is a full-time, non-exempt, union, regular position.
