Medical Case Manager

Lifelong seeks a Medical Case Manager that assists primarily medium to high acuity clients to access and engage in psychosocial, medical, homecare, and supportive services. This position conducts comprehensive assessments, develops and updates service plans, and facilitates the acquisition of all appropriate services. This is a full-time, exempt, union, regular position.
