The Practice Transformation Program (PTP) is a region-wide quality improvement process change model employing longitudinal coaching, training, and clinical assistance to selected clinics.

- 13 clinics in 10 states
- 4 Ryan White Clinics, 9 Federally Qualified Health Centers (FQHCs)
- 3 urban clinics; 10 small town/rural clinics
- Local AETC practice coaches in each state partner with clinic sites
- Regional preceptorship sites in Washington and Oregon
