HIV/AIDS Clinical Preceptorship Program for MDs, PAs, and NPs

This one- or two-day educational program is a clinic-based preceptorship for health care providers during which they will observe and work with an HIV expert preceptor. Preceptorships will be designed to meet the specific needs of individual trainees based on current levels of experience and practice settings. Preceptors will focus on the care and treatment of patients with HIV infection within the context of a comprehensive, continuum of care model. A combination of didactic and experiential instruction will include HIV risk assessment, diagnosis of primary HIV and opportunistic infections, therapeutic interventions (i.e., drug treatment regimens including managing side effects, salvage therapies, and adherence strategies), and risk reduction strategies to help people infected with HIV prevent transmission to others. Instruction will also include strategies for effectively addressing patients’ psychosocial needs related to mental health and substance use disorders and social support. Participants (preceptees) will accompany the preceptor to observe at least four work-ups with patients being treated for HIV. The American Academy of Family Physicians (AAFP) provides up to 16 continuing medical education (CME) for MD, PA, DO preceptees completing the clinical preceptorship.

HIV Clinical Preceptorship Learning Objectives

Participants will be able to:

• Diagnose primary HIV infection
• Conduct baseline evaluation of newly diagnosed patients with HIV
• Cite principles underlying Antiretroviral Therapy (ART) initiation and maintenance
• Describe adverse effects and drug interactions associated with ART
• Diagnose and manage common HIV co-morbid conditions and opportunistic infections
• Discuss and prescribe biomedical HIV prevention
• Describe how to use a multi-disciplinary team for HIV management

HIV Clinical Introduction to High Resolution Anoscopy (HRA) Preceptorship Learning Objectives:

Upon completion of this program, attendees should be able to:

• Review recommendations for screening of anal dysplasia and cancer in people living with HIV
• Describe diagnostic procedures to evaluate intraepithelial neoplasia (AIN) and anal cancer
• Differentiate between Low-Grade AIN and High-Grade AIN
• Discuss appropriate treatment and management options for AIN and anal cancer
• Identify prevention strategies for AIN and anal cancer

Please note: This is a one or two day preceptorship. The goal of this preceptorship is to provide an opportunity for trainees to learn more about the role of anal cancer screening and HRA in the care of people with HIV, as an adjunct to pursuing the required further training to become proficient at HRA and implement HRA in their practice settings.

HIV Corrections Preceptorship

This two-day educational program is a prison-based preceptorship for health care providers during which they will observe and work with an HIV expert preceptor. Preceptorships will be designed to meet the specific needs of individual trainees based on current levels of experience and practice settings. Preceptors will focus on the care and treatment of patients  with HIV within the context of a comprehensive, continuum of care model. A combination of didactic and experiential instruction will include HIV risk assessment, diagnosis of primary HIV and opportunistic infections, therapeutic interventions (i.e., drug treatment regimens including managing side effects, salvage therapies, and adherence strategies), and treatment as prevention. Instruction will also include strategies for effectively addressing patients’ psychosocial needs related to mental health and substance use disorders and social support. Participants (preceptees) will accompany the preceptor to observe at least four work-ups with patients being treated for HIV.

Corrections Preceptorship Learning Objectives

Upon completion of this program, attendees should be able to:

• Diagnose primary HIV infection
• Conduct baseline evaluation of newly diagnosed patients with HIV infection
• Cite principles underlying antiretroviral therapy (ART) initiation and maintenance
• Recognize adverse and potentially life-threatening side effects of ART
• Approach diagnosis/management of common HIV co-morbid conditions

HIV Preceptorship for Nurses

This 12.25 hours (two days) course provides experiential learning along a spectrum of care, treatment, and services for people who are at risk for and/or living with HIV. Educational goals are: 1) To ensure the quality and safety of HIV care which is delivered in a culturally appropriate manner; 2) Providers are able to effectively address the multiple and complex needs of people living with HIV.

Nursing Preceptorship Learning Objectives

Upon completion of this program, attendees should be able to:

• Differentiate between a diagnosis of primary HIV infection or AIDS
• Review concepts of antiretroviral therapy (ART)
• Provide relevant patient education
• Promote the use of multidisciplinary teams in providing HIV care
• Summarize current PEP and PrEP protocols

This one-day (8-hours) educational program, available at both Madison Clinic at Harborview in Seattle (resuming in 2023) and at HIV Health Services Center in Portland, is a clinic-based preceptorship for clinical pharmacists. Preceptorships will be designed to meet the specific needs of individual trainees based on current levels of experience and practice situations. Methods include some didactic instruction but focus primarily on case discussion, observation of pharmacist interactions with patients, support materials (e.g., pill chart), and following one patient through the Madison Clinic’s Introduction to HAART Protocol. The protocol includes assessments by the primary care provider, nurse, pharmacist and nutritionist. Completion certificates indicating the amount of time in training will be issued to preceptee. 

Pharmacy Preceptorship Learning Objectives

Upon completion of this program, attendees should be able to:

• Recognize symptoms of acute HIV infection
• Discuss considerations of antiretroviral agents in treating HIV disease
• Educate patients about drug/drug and drug/food interactions
• Explain the importance of adherence as it relates to drug resistance
• Initiate occupational and nonoccupational postexposure prophylaxis

HIV Case Manager Preceptorship Program

The MWAETC offers a one-to-two-day HIV Case Manager preceptorship at Anschutz Medical Campus (Aurora, CO) and HIV Health Services Center (Portland, OR).* Patient contact will occur through care conferences that involve the program's multi-disciplinary team, as well as individual interaction with social workers as they conduct rounds, perform assessments, address patient concerns and plan discharges. The training will emphasize various assessment skills needed to help identify and address patient conditions. Depending on the specific needs of the patients in residence, areas of concentration may include cognitive status, mental health concerns, ability to perform activities of daily living, readiness for discharge, spiritual/cultural aspects, legal issues, and family/support system interactions. Staff will provide opportunities for discussion and questions throughout the day. (*Seattle locations (Bailey-Boushay House and the Madison Clinic at Harborview) are unavailable at this time).

Case Management Preceptorship Learning Objectives

Upon completion of this program, attendees should be able to:

• List the basic qualities of an effective case management program
• Explain Ryan White Program medical case management guidelines
• Conduct a comprehensive case management assessment with patients living with HIV
• Demonstrate clinically useful techniques for addressing service needs of patients with HIV and co-occurring mental health and substance use disorders
• Address sociocultural issues associated with HIV
